About Us

Booroongen Djugun specialises in providing Community Care and Residential Care services to Aboriginal and Torres Strait Islander Elders in the Kempsey area, offering a space to access culturally-informed care. We’re also a Registered Training Organisation, offering vocational education and training to staff and community members looking to broaden their horizons and enter the workforce.

The space itself was designed with the totems of fire, water, earth, and sun in mind, in order to support the strong ties to traditional life that many clients of Booroongen Djugun hold.

Summary;

We are seeking an experienced Community Coordinator to coordinate the provision of a flexible range of services and programs within a Wellness and Reablement approach, which promotes each client’s capacity to live as independently as possible. The Community Care Coordinator is responsible for delivering daily care services to clients in accordance with the established care plans, including documentation and record keeping.  This role also coordinates the intake, assessment and review processes of all clients to ensure compliance with various funding guidelines, state and federal legislation and BDL’s policies and procedures.

Key responsibilities and duties

Client Liaison 
• Provide high quality service at all times with respect to the diversity and complexity of client needs.
• Provide a flexible service response for clients whereby services are organised around the client and their carers, particularly in respect to client-worker match and times/duration of available service in a client goal directed approach. 
• Provide support for initial assessments and documentation of new clients by responding to enquiries and referrals from My Aged Care and external agencies. 
• Maintain effective monitoring systems to identify changes in client needs and initiate appropriate referral for reassessment or review. 
• Liaise with Aged Care Package case managers when client/carer feedback or changes are required. 
• Maintain effective communication with all clients and other service providers. 
• Where required, assist clients with their day to day service issues, including the escalation of client service issues or complaints as necessary


Service Delivery Administration and Financial Management
• Work with clients to create their initial individualised Care Plan and Budget based on their package level. 
• Ensure ongoing operation within client’s individualised budget and support the preparation of relevant acquittal forms for funding agencies
• Maintain accurate information in the MYP database to deliver customer, provider, financial and service delivery reports for funding bodies and BDL as required. 
• Provide timely statistical and other data to the Executive Manager to monitor service outputs and to meet internal and external data collection requirements 
• Develop and implement changes to support the implementation of flexible Wellness & Reablement Model service responses.  Identify and implement ways to increase service provision within existing funding streams to maximise grant budget utilization by monitoring income and expenditure to meet or better the service.
• Request client health summary from doctors and other health professionals and forward client referrals/requests to allied health professionals, e.g. podiatry, physiotherapy, OT. Be proactive in encouraging clients’ wellness, reablement and independence and the provision of holistic care.
• Establish and maintain contact with clients, their carer’s and identified target groups in the community. 
• Undertake any duties or customer enquiries that enhances any service or function. 
• Coordinate the development and administration of a regular Quality review against the requirements of the Standards, and participate in operational audits.
• Participate in local and regional meetings and forums, and attend appropriate peer networking events to enhance knowledge and information in relation to the position.
• Have clear understanding of SIRS and RIs and obligations to report
• Report and take appropriate action for your level and authority on all complaints following the feedback and complaints Management and resolution System ensuring mandatory reporting requirements are met.

Core Competencies/Requirements/Experience
• Relevant qualifications and experience in community aged care and/or disability services. 
• A demonstrated understanding of community care issues, and experience in the preparation of client care plans and relevant documentation processes to support consumer directed care. 
• A thorough understanding of CHSP, HCP and NDIS and the wellness and reablement principles.
• Proven ability to communicate effectively, non-judgementally and empathically with older people, people with disabilities and their carers and the ability to provide friendly, positive and supportive customer service to clients in a diversity of circumstances. 
• Awareness and support of the philosophies of healthy ageing and Wellness and Enablement
• Sound understanding of aligning budgets with program delivery targets
• Highly developed communication, interpersonal and problem solving skills 
• Highly organized, able to prioritise tasks and work to a deadline

Position Requirements

All applicants are required to undergo a NDIS Workers Screening Check and a Working With Children Screening Check. As a Residential Care Facility, Influenza vaccination is mandatory for all staff who work onsite.
